Johanna started practicing boxing at the age of 13 and turned professional at 14, always under guidance of her father and trainer Ruddy Peña.
Peña-Álvarez made her pro debut at the age of seventeen, on June 29, 2000, winning a 2nd round TKO over Natali Lara. She won her second pro bout, before losing a 6-round unanimous decision to Gisselle Salandy on August 8, 2000. Peña-Álvarez would go 12-0-1 in her next 13 fights, before losing a 10-round decision to WIBF Flyweight Champion Regina Halmich on January 17, 2004 in Germany.
